I was in the process of writing a message almost identical to Fred's. He beat me when I went to get some coffee!

I tried the run-in approach yesterday because that's what we do in the standard TOC. However, the commands and command descriptions are treated as independent paragraphs rather than as pairs, so the things that are on the same page were not necessarily next to each other in the alphabetized list. Sometimes there were half a dozen run-in paragraphs strung together, followed by one or two single entries, followed by another bunch of run-ins (you get the point).
