I'm still confused. Say my husband, the programmer, works onsite and gets paid Corp to Corp where the "recruitment" firm requires Liability insurance. The husband falls down a flight of stairs at the client's office and he sues his own insurance company? Or does he sue the recruiter? Or the client?

He wouldn't sue; he'd file a claim with his insurance company. This is all about protecting the client company from massive jury awards for "slip and fall" injuries. You sign away your right to sue the client company, apparently. It's like health insurance more than accident insurance.
