The type of Train-the-trainer course depends on what you're trying to accomplish.
In a previous life, the Train-the-trainer courses I attended concentrate on taking someone who is an expert in the subject and teach them how to train others. More time is spent on training techniques than on the systems that the trainer will be explaining.
The Train-the-trainer course at my current employer follows the same assumption: you are taking an expert and teaching them to train. That type of course is usually no more than a day in length.
On the flip side, twice I, as the trainer, needed to train people to become the experts. These newly minted experts needed to go forth and train others -- because I couldn't be in three states at once (in the days before WebX and video conferencing). Those courses took 3.5 days, and were VERY technical (with lots of handsout, hand-on training, etc.), plus I had to slip in the "how to train" part. I always felt the "how to train" portion got shortchanged, because the technical aspects weren't always ready for primetime, and short training timeframe in general.
