Depending on how you're designing the help system, you might want to consider a link to a popup or dhtml (if you're doing HTML help) that gives the keyboard shortcuts. Serves teh same purpose as an appendix would in the user manual. It also saves a bit of screen real estate, by not including all keyboard and mouse options for every procedure. The users will probably get the idea fairly quickly that if they want info on keyboarding, it's just a click away. However if they're looking at procdural help, they're likely more interested in what happens, and can make the jump from mouse to keyboard concepts.
In my experience, if you have both keyboard and mouse options, advanced users tend to go keyboard, rather than the other way around. (e.g., I never click the Save options on the toolbar or menu bar, I just do CTRL+S).
