Ms. Ruiter's method will work (although I'd be nervous about losing
existing headers and footers during the process), and you should use it
if you absolutely *have* to put all the chapters into a single file.

Do you know, though, that you can easily get a book-wide TOC and index
*without* combining the chapters into a single file?

In the file that contains your front matter, insert a section break and an RD field for each additional chapter, something like this:

title page
copyright page
--- page or section break ---
TOC field
--- odd-page section break ---
{ RD chapter01.doc }
--- odd-page section break ---
{ RD chapter02.doc }
--- odd-page section break ---
...
--- odd-page section break ---
{ RD appendixNN.doc }
--- odd-page section break ---
{ INDEX }

This works great for what you want to do.
