We do not have PDFMaker installed (and prefer it that way). And of course
we'd prefer not to have to create the links ourselves in Acrobat.

Why in the world would you prefer not to install the tool recommended by Adobe for precisely the task you need to perform...especially after buying it?

PDFMaker is simply a Word macro that provides an interface to several PDF-generation tasks and assures that they're performed in the correct order.

I've been using the PDFMaker included with Acrobat 4 and later Acrobat 5 to create PDFs from Word 2000 for about four years now.

It works great, and makes simple the things you mention wanting to do.
