duplicate index entries in Word XP

The archives did not turn up anything, so here goes.

In Word XP, I am getting duplicate index entries, instead of putting all
of the page numbers for a unique entry on the same line.
For example:
Deferrals 8
Deferrals 13
Deferrals 53

Should be of course:
Deferrals 8, 13, 53

Where is the button that sets up "please put all pages for the same
entry on the same line"?
