I used to embed some of my comments for reviewers as Hypertext markers in
FrameMaker. The markers later converted to notes when distilled. This
allowed me to make my comments while entering the text.

I used one of the TimeSavers hypertext marker shortcuts to do this. I later
used FrameScript to setup a hot-key combination that automated the whole
process to the point where I entered a simple hot-key combination, then
typed my comments followed by the ENTER key. It took a while to setup the
first time, but was well worth the effort.

When I wanted to remove the comment, I just deleted the marker.
