If I remember correctly, the problem arose from the fact that 216 indexes (colors in the cube) were reserved for the colors you can define using triplets of 00, 33, 66, 99, CC, and FF hexadecimal.

If you use only those colors, you could be assured that the browser would display the color you intended.  That's why it was called the "safe" palette (and I don't think it was specific to Netscape).

If you used any other colors, though, the browser had to swap out colors from the safe palette to make room for them.  Since you had no control over which colors got swapped out, you couldn't be sure what would be displayed when a particular color was called for.
