Jan wonders: Is everybody who signs off 'responsible' for everything in
the documentation, or is the signing off limited to certain areas,
e.g., an SME signing off only on factual correctness?
Engineering and QA are only responsible for their areas of expertise. But at the end of the day Product Management is "responsible" for the entire product (including documentation); their sign-off comes after the third and theoretically final review.
Even then it's not supposed to mean that they think the doc is perfect, just that they've reviewed it and any remaining issues (that aren't show-stoppers) have been noted for future updates. You would think they had to sign it in blood, though. :-)
And yes, of course, Documentation is *really* responsible for all of it. <g>
