I think we're alot smarter than Andrew gives us credit for. I'd say at least 19% smarter.
The real reasons why innaccuracies get in the docs are:
20% misunderstanding (didn't quite get it but thought I did)
20% laziness (don't wanna bother fixing minor errors)
20% hubris (too proud to ask for help)
20% ennui (bored with the subject -- move on)
19% poor time management (too busy reading email)
1% programmer's fault/bug
