It's a character set issue. You can fix manually by just editing the bookmarks to delete the boxes. I think that if you look at your source document with invisible markers turned on, you will find things like hard returns, non-breaking spaces, etc., where the boxes are.
You can probably fix the problem permanently by switching to a non-Unicode font for your heading style. The bookmarks are, IIRC, straight ASCII. Or maybe ISO 8859-1, but nothing past that.
