Okay, fair enough, given the way I stated my case. To try and clarify -- I have been lead to understand that matching word-for-word and including all headings at the chosen level are *generally* considered requirements. I acknowledge that they aren't always followed to the letter, but if one departs from them, one does so in very specifically-defined situations, and along very specifically-defined lines. The INTERCOM article seemed to treat both requirements with the attitude, "If you wanna do this, fine. If you don't wanna do this, fine."

David...you contradict your own statement on both points...Example from the
second point:

If you include all headings, then there are no exceptions. If there ARE
exceptions, then you are agreeing with the STC contradiction.
