So I think that sleeve needs a prominent warning on the order of "YOU
MUST RETURN THIS DVD IN THIS SLEEVE BECAUSE THE SLEEVE TELLS US WHO
RETURNED IT, IF YOU LOSE THE SLEEVE< YOU MUST ENCLOSE A NOTE ... [etc]"
What does everyone think?

Actually, this warning is on their website, I believe under the FAQs. We have a subscription for 5 DVDs at a time, and I have lost two or three sleeves in the last year. If the warning were on the sleeve, you might not bother to read it until you lose the sleeve --oh, then you couldn't read it, either :)
