I've run into this problem before. Even if you update
the text inset, the ?? remains. I believe this is how
we got around it:

Edit > Update References
Select All Xrefs and both Text Inset options
Click Update

See if that does the trick. Unfortunately, you may
have to repeat the step during the book updating
process (I don't remember whether you do), so I always
search a PDF version for ?? to make sure I got 'em
all.
