If it's reference material about an element, I'd do an intro paragraph (if possible) then the grab, then the illustration. A brief intro gives the reader a clue as to what the graphic is, why they should be looking at it and how looking at the graphic will make their life happier. :-)

If it's in a tutorial, I'd put the grab just below the step. (Click X button (following graphic) <RETURN> grab).

If its in conceptual information, place it within the flow of the text in a logical place or at a paragraph break, following the description.

That's my two cents.
