How do errors get in my work?

1. I messed up, and/or
2. My SME/reviewer(s) messed up.

Now, the REASON for that mess up can be a bunch of different things, but the
bottom line is that somebody messed up and *I* need to fix it. I don't think
I need a case study for that.
