It is not a myth that programmers sit and figure out someone else code. That is in fact what they are paid to do more than write code. They write very little code, but it must be fully integrated with other people's code. Even API doc is inadequate.

Yes, it is a waste, but companies don't pay programmers to document their code, so they don't. Practice is changing extreme programming is changing this, as is the free software movement. The free software movement incorporates high-level code reviews into their practices. The point of the review is to talk about all the alternatives, how the implemented alternative was selected, and the history of the alternatives attempted and their experimental results. Much more gets communicated than just code or interfaces.
