Word's online help has:
"{ FORMCHECKBOX }
{ FORMDROPDOWN }
{ FORMTEXT }
These fields insert a check box, a drop-down list, or a text box in a form. You
cannot insert or modify these fields directly. They're inserted by the Check Box
Form Field, Text Form Field, and Drop-Down Form Field buttons on the Forms
toolbar. To edit these fields, you must use the Form Field Options button on the
Forms toolbar."

(Unfortunately, if you go in the index to "check boxes, inserting" in the online
help index, it doesn't give you that information. Instead, it gives you a page
of information about checkboxes on web pages, which is not helpful if you're
creating an ordinary Word doc.)

View > Toolbars > Forms will display the toolbar.

That's if you want a *working* checkbox. If you just want a box that you can
check off with a pen after you print the page, you can do
Insert > Symbol, change the font to Wingdings, and browse around to find the
checkbox symbol. (But your users have to have Wingdings installed to see the
checkbox then.) Or you could insert a small graphic.
