Sorry for the late reply Kristin, but I noticed you didn't get many others
so thought I'd throw my pennies in. PDF Fill-In Forms sound like they would
fit the bill nicely. The IRS uses them extensively for this purpose, check
them out at: http://www.irs.gov/forms_pubs/fillin.html and see if this is
what you're after.

If you don't want to create them all yourself, check out this site. They do
PDF form creation:
