Hmm...must be all that clean living that I do. As an amateur student of typography, I routinely think about design on the job, and I can't remember the last contract in which the client DIDN'T praise the design. True, nobody has swooned, and there's a lamentable lack of groupies (or maybe not, since I value a quiet home life. However, several times, I have had what I consider the ultimate compliment: clients who have said that they didn't care about design admitting that my very limited expertise had resulted in a document that was more usable or that represented the company effectively.

Obviously, solid design needs to be backed by solid writing. Still, I think that clients can appreciate it more easily. After all, to compliment content, they have to spend time reading it.
