Thanks to everyone who responded to my message about questionnaires. Unfortunately, the responses I have received haven't really answered my question. I am not interested in learning how to write a questionnaire; I'm already working with a well-developed set of questions. What I'm trying to do is make the thing look good. One of the issues is this: we'd like to come up with a numbering system for the questions, but we're not sure what to do about numbering headings (which would help with navigation -- important for the teams of harried people who sometimes collaborate on a single questionnaire -- but which would violate our style). Also, in our template for procedural documents, the body text has a fairly substantial left margin (about 2 inches), with headings that are closer to the binding (about 1.25 inches from the edge of the page -- don't know the name for this). My instinct is that this type of design is not really appropriate for a questionnaire, but I'd like to l!
ook at some other questionnaires that are out there before deciding. Which brings me to my request: can anyone send me a copy of what they think is a well designed questionnaire, or comment on some of these design issues?
