I think what you're looking for are radio buttons. You can create them on
an HTML form using a regular INPUT tag, but with the TYPE="radio". Any
radio buttons with the same name (NAME="gender", for example) belong to
the same group, so they are exclusive within that group (that is, if one
is selected, the other is deselected automatically).

So the first question on your form (I found the Web site easy to use)
would have two INPUT tags following it, with the same type ("radio"), the
same name ("question1") but with two different VALUE elements
(VALUE="yes" and VALUE="no"). Beyond that, you can control other aspects,
such as which radio button is on by default (selected="yes" for the
default selection). Beyond that, check out any HTML reference guide for
more details...
