The only ones with which I'm familiar are the plans available from IEEE and from the National Writers' Union. The latter uses Aetna/US Health Care, a POS rather than HMO. All I've needed were checkups and periodic referrals to my ophthalmologist for routine checkups, but I've been very satisfied.
