Of the specialist agencies I've had good experiences with
Kudos (as both candidate and employer). You might also try
www.digitext.co.uk. Both do permanent and contract. Can't
help with the US bits of your question.

Of the web listings I found www.jobserve.co.uk gave the most
tightly focused results. This is where I found the position
I've just taken.
