Doc-to_Help from WexTech provides templates you can use for printed
multichapter (multifile) manuals. The Master documents in these
templates work much better than Word's built-in templates. If you must
work with Word, you could try using these templates (the software's
not cheap - Doc-To-Help 2000 $499, Doc-To-Help 2000 Upgrade $249).
(Even after using these templates Word shortcomings get in the way.)

FrameMaker is excellent at handling long multifile documents. Xrefs
across separate files/chapters are easy to create, and they don't
break. FrameMaker's not perfect - some features are hard to get your
hands on - but I'll take it any day over Word for large projects.
