Not that I'm an expert, but...

When you install the full Acrobat, make sure to include PDFMaker in the installation options.

In Word, you'll see a new command on the File menu and a new one-button tool bar (the button has the Adobe Acrobat logo, a stylized red script "A"). Use the command or the tool to start the process.

For making page numbers in the index hyperlinks to the corresponding pages, you'll also want a tool called Sonar Bookends Activate ($195) from Virgina Systems. This nifty little tool will save you an hour or so of mind-numbing drudgery every time you create a PDF file.
